The filled form will also be displayed at www.karmayog.org/roko along with any feedback that is received from the concerned government authorities. A: Details of the government organisation 1. Name of government organisation: Ration Office 2. State: Maharashtra 3. City: Mumbai 4. Department: Name Change Dept 5. Location: Mulund West 6. Designation of government officer involved: Junior 8. Address of the government organisation: 35/e, Pwd Compound, Mulund (West), Mumbai - Tel no: +91 22 22621791 B: My Complaint Details 1. Period of incident: 2008 - Nov 2. Services that were sought: I married on 10th July, 08. I have received my marriage certificate. I have all my requierd documents for adding my wife`s name in my ration card.But,He was asking me 1,000 Rs for adding name of my Wife. He then directed me to an Agent saying that if you go to him, we may do it for Rs.500. 3. Problem/s I faced: They said, if no bribe then, it may take one month or six months for name change 4. Bribe amount: 1000 5 a. Was the bribe paid? No....But, If no action is taken in 1 week, I`ll have to pay bribe, since it is urgent for me to add my wife`s name in my ration card 5 b. Could the work have been done without paying the bribe? If so, how? not yet 6. Result: nothing C: Additional Informtion attached Details of Content: Nil D: My Suggestions 1. For head of government department: Suggestion 1: Transperancy in all the processes. Legal action against Rationing officers. 2. For the Anti Corruption Bureau (ACB) / Central Bureau of Investigation (CBI): Suggestion 1: Using IT (Software) for Rationing office. IT enabled name change process, adding name process, issuing of ration cards, etc. 3. For other citizens: Suggestion 1: Dont give bribe. Act against them. Take their pictures from your mobile camera.
